If your store is changing its name, please attach a photo of the new signage official (preferably government issued) documentation that reflects the new name. Please make sure that the owner name, store location, and new store name are all linked on the documents. Please do not send documents sensitive information such as social security numbers, bank account information, or photo IDs.
